If you would like to see if your insurance company will reimburse you for a portion of the assessment, give them a call and ask if you will receive out-of-network reimbursement for a psychological evaluation. 

Below are sample invoices. Provide your insurance company with each of the CPT codes listed and they will tell you if you will get reimbursed for each code. The fee per code is listed so you can calculate about how much they plan to reimburse you.
